A survey aimed at middle school children asks kids about gender identity, climate change anxiety and even suicidal thoughts — and warns parents to “not share the survey with anyone else.”
Thomas Blake Middle School in Massachusetts said the goal of the survey “is to improve student health and safety” and sent it to parents in advance, offering them the option to opt their child out. The questionnaire obtained by Defending Education asks middle schoolers about their sex and gender identity, giving them options like “non-binary, gender-queer, gender-fluid, gender non-conforming.”
It also blatantly asks “Are you transgender?”
“What sex were you assigned at birth, on your birth certificate?”; “Which of the following best describes your gender?”; “Some people describe themselves as transgender when their sex at birth does not match the way they think or feel about their gender”; and “Are you transgender?” the survey asks.

Middle schoolers are also asked about their sexual preferences.
Students are then asked if they have ever considered suicide or if they harm themselves.

“How often do you worry or feel stressed about climate change?” the survey continues. “Do you think you are personally being affected by climate change? Have you taken any actions to help reduce the effects of climate change?”
Medfield Public Schools, which oversees Thomas Blake Middle School, did not respond to the Daily Caller News Foundation’s request for comment.

“These survey questions are obviously inappropriate for 11- and 12-year-olds,” Erika Sanzi, senior director of communications at DE, said in a statement. “To the school’s credit, they provided a copy of the survey to all parents in advance. It would be better, however, if they would just stop asking these intrusive and ideology-based questions.”
